Providing Daytime Care for McCreary’s Elderly and Disabled

McCreary County’s Horizon Adult Health Care Center and its employees have moved from the previous location in the Big South Fork Medical Plaza to a newly renovated section of the Oaks Plaza in Whitley City. McCreary County Horizon Center Nurse Director, Michelle Coffey, is ecstatic about their move.
“We are here to assist the elderly of McCreary County,” Coffey stated with a smile. “And we are definitely enjoying our bigger and brighter center.”
As an adult day health center, Horizon serves the elderly (ages 65 and above), the blind, or disabled including (but not limited to) individuals suffering from Parkinson’s disease, dementia, post-stroke, and other age-induced physical and mental ailments. The facility can provide assistance for those who need only occasional care to those who would otherwise be candidates for a nursing facility. In addition, door to door transportation is provided to and from the Center via RTEC, and the Center is equipped with a van to transport clients to local doctor visits, on outings, and to public access areas to promote fun and educational social experiences. The center is open Monday thru Friday between the hours of 7:00 a.m. until 5:30 p.m.
At Horizon, traditional nursing services are provided during daytime hours. Participants return home to their families in the evening. This arrangement allows the participant’s family members to fulfill their daily responsibilities and provides aging loved ones with opportunities for socialization and rehabilitation. The center provides a daily breakfast, lunch, and snack. Recreational activities include free WI FI, birthday parties, arts/crafts, bingo, and other activities. The McCreary Horizon Center also features a pool table, ping pong table, and two televisions. Participants occasionally go on trips to the movies, bowling, etc. and are offered tri-weekly outings to shop, pay bills, etc.
Horizon provides skilled nursing care and rehabilitation services including bloodwork, monthly head to toe nursing assessments and weekly vital sign assessments including blood pressure and blood glucose checks. Horizon offers a full staff with a registered nurse and a licensed practical nurse present as well as multiple nursing assistants. Nurses provide assistance with medication regimen, prefilled weekly medication boxes, prefilled insulin syringes, and communicate with doctors’ offices concerning patient care. Other available services include medical monitoring, dietary consultation, and memory enhancement. Assistance with activities of daily living activities such as bathing, grooming, laundry, eating, and dressing are provided as needed.
Horizon accepts several payment sources including Medicaid, private insurance (those with adult day coverage), and private pay. The enrollment process consists of three basic steps: referral, assessment, and plan of care. First, a referral is received from a hospital, doctor, friend, family member, etc. to the patient care coordinator, Lyndsey Vanover. The patient care coordinator schedules a time to visit the potential client or for them to visit the Center. This gives the patient care coordinator an opportunity to assess the potential client’s needs. Finally, if accepted, a plan of care is developed by the nurse, and the participant is admitted.
